In vitro maturation (IVM) technology facilitates the usage of immature oocytes, which is clinically important for the aged women. However, the maturation rate is low for the oocytes from the aged women. adipose tissue-derived MSCs (ADMSCs), as important mediators of intercellular communication, have been widely used to restore ovarian function and improve female fertility. In this study, we isolated AD-MSCs-derived exosome (AD-Exo) and collected the human immature germinal vesicle oocytes, and we added these HUCMSCs-exosomes to the conventional IVM culture system. This study aimed to evaluate the effects of ADExo on quality oocyte in vitro maturation and parthenogenetic embryo development. We obtained AD-Exo through mixed-method ultracentrifugation and size-exclusion chromatography. Transmission electron microscopy revealed an increase in the expression of exosome markers in the first four of thirteen fractions. The highest maturation rate of oocytes treated with AD-Exo was observed. Moreover, increased expression of Gdf9 and Bmp15 was observed. The developmental rate was the highest upon treatment, which increased the total cell number in blastocysts. Embryonic development to the 2-cell stage was similar between the control and ADExo groups; however, development to the 4-cell stage and blastocyst was significantly increased in the AD-Exo group compared with that in the control group (P < 0.05). Based on the results, we propose that AD-Exo may provide a novel and cell free strategy in the improvement of the IVM in elderly infertile women in the future.
