Background & objectives: This study investigated the effect of regular physical activity on serum androgen levels and erectile function in men over 50 years of age. Materials and Methods: In this quasi-experimental study, 33 men with a mean weight of 79.2±8.5 kg and BMI of 26.13±3.3 kg/m2 were randomly selected. Subjects were randomly divided into two groups: control and concurrent training. Serum levels of dehydroepiandrosterone (DHEA), estradiol, erectile dysfunction, and other dependent variables were measured 48 hours before and after the training period. Data were analyzed using analysis of covariance (ANCOVA) (P = 0.05). Results: The results showed that serum levels of DHEA (P=0.001) and estradiol (P=0.01) in the simultaneous training group showed a significant increase compared to the control group. The results also showed that sexual performance in the indices of erectile function (P=0.039), orgasmic function (P=0.01), and total scores (P=0.004) showed a significant increase in the simultaneous exercise group compared to the control group. Conclusion: These findings indicate that regular physical activity can be considered an effective intervention in improving the levels of DHEA and estradiol, sexual function of men over 50 years of age, and emphasize the importance of exercise in maintaining the quality of life and sexual health of older men.
