Abstract

Introduction:Acromegaly is usually associated with changes in carbohydrate metabolism from baseline glycemic change to diabetes mellitus (DM). This study aimed to evaluate the concentrations of TNF-α, IL-10, IGF-1, Insulin, HOMA-1R, Cholesterol, TG, LDL, HDL, VLDL, AST, ALT, AP, and to investigate the association of these parameters in diabetic acromegaly disorder. Furthermore, the study also includes Cytomegalovirus (CMV) detection and its association with other parameters which have been analyzed such as Lipid profile test) (LFT). Material and Methods: Detection of CMV infection was performed by the PCR method. The serum concentration of TNF-α and IL-10 in patients (n=50) and healthy group (n=50) were measured by ELISA technique. The results of the assays in the control group and patients were compared by statistical analysis with a significant level of p < 0.05. Results: Diabetic patients with acromegaly exhibited insulin resistance due to significantly elevated IGF-1, Insulin, and HOMA-1R. Increased Lipolysis activity is associated with diabetic acromegaly disorder due to elevated Cholesterol, TG, LDL, and VLDV. Increase in the level of the TNF-α and IL-10 compared to control inflammatory responses which were significantly correlated to insulin resistance and diabetic acromegaly. Liver function was revealed to be involved in the pathogenesis of diabetic acromegaly due to significantly elevated ALT, AST, and AP activities in patients. Comparison of CMV-positive samples in healthy (1.9%) and acromegaly groups (23.5%) by PCR findings and significant elevated TNF-α and IL-10 in CMV-positive samples (compared to negative ones) could show that patients with acromegaly are prone to viral infection. Conclusion: The result of the present investigation suggests these two cytokines as useful immunological markers for diagnosing acromegaly.
